NOT A TERMINATION REQUEST

Form must be submitted at least 5 business days before next draft date for changes to be effective for that draft date.

Per Atlas policy, all members have the option to put their membership on hold one (1) time per contract period for up to three (3) consecutive months, with a minimum of one (1) month, at no charge, for any reason. Upon expiration of Hold Date above, your account will automatically become active and payments shall resume. Should you elect to cancel during the Hold Period, you will be subject to the 30-day Cancellation Policy per your Membership Agreement. I also understand that if I would like to put my membership on hold more than one (1) time per contract period or for an extended period of time outside of Atlas policy, my current membership will be terminated and I will be subject to current rates upon return to regular membership.
